3.2 During the said period, enumerators of the Consumer Search visited each booth in the three halls, delivered questionnaires to each eligible exhibitor (i.e. those exhibitors who sold or promoted prepackaged food products during the Food Expo 2011) by hand, and invited the exhibitors to complete and return the questionnaires to the enumerators on or before 15 August 2011 by hand, fax or email.

Completion and return of an instrument appointing a proxy will not preclude a shareholder from attending and voting in person at the above meeting.

5) For cancellation of registration in the Road Cargo System, the responsible person has to send this completed and stamped Information Change Form (CI(E)) in person to any one of the Registration Centres listed below with (i) copy of his / her HKID Card / Passport, (ii) authorization letter (issued by the proprietor or a partner or by Board of Directors) to appoint the registered responsible person unless he / she is the proprietor or a partner or a director and (iii) copy of Business Registration Certificate / a stamped supporting letter (issued by the head of organization with organization name and address) roving exemption of business registration (for organization only： e.g. schools). p

Every candidate is required to deposit a copy of the completed consent forms with the relevant RO and to notify the RO in writing of any revocation of consent.

Whether or not you intend to be present and vote at the AGM, you are requested to complete the enclosed form of proxy in accordance with the instructions printed thereon and return it to the Company’s Hong Kong branch share registrar, Computershare Hong Kong Investor Services Limited, at 17M Floor, Hopewell Centre, 183 Queen’s Road East, Wanchai, Hong Kong as soon as possible and in any event not less than 48 hours before the time appointed for the holding of the AGM or any adjournment thereof.

Application for all or any part of a Qualifying Shareholder’s provisional allotment should be made by completing a PAL and lodging the same with a remittance for the Rights Shares being applied for.

To be valid, the form of proxy duly completed and signed in accordance with the instructions printed thereon together with the power of attorney or other authority, if any, under which it is signed or a certified copy thereof must be delivered to the office of the branch share registrar of the Company, Computershare Hong Kong Investor Services Limited at Shop 1712-1716, 17th Floor, Hopewell Centre, 183 Queen’s Road East, Wanchai, Hong Kong not less than 48 hours before the time appointed for holding the meeting or any adjournment thereof.

7.1 A TUE will only be considered following the receipt of a completed application form that must include all relevant documents (see Appendix 1 – TUE form).

(f) The applicant should return the completed application form together with supporting documents to the kindergartens/child care centres concerned as soon as possible.

11.2 To protect personal data from unauthorized disclosure, the applicant should enclose the completed application form, the supporting documents and other required documents in an A4sized envelope which is sealed and signed for submission to the School.

A form of proxy for use at the Annual General Meeting is enclosed with this circular and such form of proxy is also published on the websites of the Stock Exchange (www.hkexnews.hk) and the Company (www.computime.com). To be valid, the form of proxy must be completed and signed in accordance with the instructions printed thereon and deposited, together with the power of attorney or other authority (if any) under which it is signed or a certified copy of that power of attorney or authority at the Share Registrar of the Company in Hong Kong, Computershare Hong Kong Investor Services Limited, at Rooms 1806–7, 18th Floor, Hopewell Centre, 183 Queen’s Road East, Wanchai, Hong Kong, as soon as possible but in any event not less than 48 hours before the time appointed for holding the Annual General Meeting or any adjournment thereof.

9.1.10 The project applicant may be required to complete and return a post-project evaluation questionnaire for its project to report on the efforts in publicizing the project and the comments of the project by the beneficiaries, and to provide quantitative measurement on the benefits to the creative sectors concerned or the creative industries as a whole, and adoption of the project achievements by the industry.

(i) if served by post, shall be deemed to have been served on the day following that on which the envelope or wrapper containing the same is posted and in proving such service it shall be sufficient to prove that the envelope or wrapper containing the notice was properly prepaid, addressed and posted (in the case of an address outside Hong Kong by air-mail postage prepaid where air-mail posting from Hong Kong to such place is available) and a certificate in writing signed by the Secretary or other person appointed by the Board that the envelope or wrapper containing the notice or other document was so addressed and put into the post shall be conclusive evidence thereof

Should QAIHT companies wish to initiate SPP applications on behalf of their new employees who are to undergo a QAIHT course, they should submit a ‘Notification of Employment and Scheduled Training’ proforma, QAIHT Form 1 Pages 1 and 2, to PLO together with the duly completed application forms and prescribed fees.

The forms can be completed in your home language and returned to your school’s main office, the SFUSD’s Central Office (555 Franklin Street, first floor lobby) or Student Support Services (555 Portola Drive, Bungalow #1).

In order to be eligible to attend and vote at the 2013 AGM, all completed transfer documents, accompanied by relevant share certificates, must be lodged with the Company’s Hong Kong Registrar and Transfer Office, Tricor Secretaries Limited of 26th Floor, Tesbury Centre, 28 Queen’s Road East, Hong Kong, for registration not later than 4:30 p.m. on Monday, 20th May 2013.
