In the Medical Services Section, the establishment of 15 additional national posts (6 National Officers and 9 national General Service staff) is proposed, as the Section has over the past years expanded the services it provides, which include round-the-clock operation of the UNOCI headquarters clinic, which is now functioning at level I-plus rather than the previous level I; patients are followed up at their homes after discharge from level III hospitals; patients are regularly visited in the 11 level III hospitals in Abidjan and in Sector West and Sector East; doctors in the regional headquarters perform outreach activity by visiting remote offices twice a week; support is provided to the Government during important functions and to the United Nations country team as part of the United Nations integrated framework; and medical support is proposed to be provided eight hours a day at the logistics base in Koumassi as opposed to the current two hours a day.

The key factors that facilitated the smooth implementation by SIAP of all planned activities for 2009 were: (a) the strong support of the host Government, Japan, which continued to provide the bulk of cash contributions, administrative and infrastructure support, and fellowships for all four SIAP-Japan International Cooperation Agency (JICA) Tokyo Metropolitan Area (TMA)-based training courses; (b) the strong support of many other member States: cash and in kind contributions from those member States increased in 2009 compared with their level in 2008; and (c) close collaboration with United Nations agencies and international organizations, which provided the opportunity to combine efforts and address effectively the diverse capability-building needs of developing member States in the region.

Ms. Ghanea drew five preliminary conclusions: (a) the discussion on limitations to freedom of expression could not be dissociated from the overall obligations under the Covenant as a whole, such as full equality before the law, due process and rights of minorities; (b) random or orchestrated acts of violence that have no reasonable link to expression should not justify limitations to freedom of expression; (c) where hatred itself incited to violence, it showed a broader pattern of violations; triggering article 20 demonstrated a failure of the State to guarantee the right to non-discrimination under article 26; (d) article 20 also required a careful and calibrated range of sanctions; at a minimum, it should not infringe expression itself and at a maximum it could trigger sanction; care had to be taken for the limitations not to have a chilling effect on freedom of expression in general; and (e) other actions in the national sphere were needed.

Improving public services and social support will be an important factor in reducing this negative dynamic, and some specific poverty alleviation interventions, such as improved maternal and child health care, or school feeding programmes, can have very positive effects relatively quickly.31 However, for lasting changes and more stable human–resource relationships, this has to be done as part of an integrated approach, one that also includes a fuller understanding of the role of fishing as a “last resort”, the causes and dynamics of people leaving and entering fishing, evolving links between rural and urban populations, markets and economies, and the political weight related to these.
